Our payment policies are designed to keep your costs as low as possible while providing you with the best rehabilitative care. For routine visits and charges not covered by your insurance, we ask for payment on the day of the visit.

For patients without insurance or procedures not covered by insurance, payment plans will be arranged at the time of service.

Our office will supply you with a superbill so you may file your insurance claim and be reimbursed for your expenses directly from your insurance carrier. Or, our office will send out the claims for you. Although our office will monitor the status of your claim, you are responsible for the follow up of your own claim. Please remember that you, not your insurance carrier, are ultimately responsible for payment. We are participants in most insurance companies PPO Networks, Managed Care Groups and Medicare. Because many plans carry a variety of deductibles and co-payments with them, out of pocket expenses may differ. At the time of each visit, deductibles and/or co-payments will be collected according to individual plans. Your deductible and co-payment will be determined through insurance verification on your first visit.
